Abstract

Degeneration of adjacent spine segment after lumbar fusion is very important problem in spine surgery. In recent years me have many articles about this question, but results of investigation far from the correct knowledge. Material and methods. Experimental part: investigation different materials and implants for spine fusion (ceramics, bone, titanium) and localization screw in vertebral body for influence it for adjacent degeneration. Experiment has been make on the rats and rabbits. Analysis of date of finite elements, mathematical model for force in adjacent segments of spine. Clinical investigation had been make in two groups of patients after lumbar fusion (152 patients).Results. Used different materials for interbody fusion on the rats shew that the minimal change in the adjacent disc had been in group with implants with higher osteointegration possibility.The best location place for screw in the body we found the central part between the adjacent discs. The minimal histological change were found in of adjacent discs after fusion in cases used interbody implants together with right screen fixation.Research of fellow up (9,4 ±1 years ) after spine fusion about the disk factor as for screw location, interbody implants, quality of fusion, lumbar lordosis. Conclusion. The results of experimental and clinical investigation has been shown that many factors influence for adjacent vertebral segment disease but the main it is quality of fusion.
